Employee Injury and Exposure Reporting

Any time an employee is hurt while working, whether they choose to seek medical care or not, they must complete an Employee Injury and Exposure Report. We have added this as an online fillable form to help make it as convenient as possible to get this completed.

If your injury requires medical treatment, leads to you missing work, or being unable to perform any of your regular work tasks, you will require clearance from HR prior to your return.
